The Rotary Clubs of District 5000 will be divided into three award divisions by club membership size. First and second place winners will be selected within each division. Club size will be determined by the beginning membership, as of July 1, 2007.

For each award, the club president will complete a form that will require answers to specific questions. Club award submissions will be limited to this form plus an additional page if an essay is included.
Clubs' submissions will be submitted to the appropriate District Chairperson or the District Awards Chairperson, as instructed on the submission form. At the District level, they will be judged within each club size group by the appropriate District committee, using a common grading criteria.
The winning club for the Club of the Year Award in each club size division will be determined by aggregating points earned by the winning clubs in the following award categories:

GENERAL AWARD SUBMISSION RULES:
- Complete one award form for each project within each Award category
- Submit an essay of no more than one page in support of each project within an awards category. Multiple projects can be submitted for each awards category. Use 8 ½ X 11 inch white bond paper. Do not use the award form for essay space. Use Times New Roman font (12 Pt).
- The submissions may contain no logos, graphs, nor letterhead but may include a digital photograph of the project suitable for inclusion in a digital audio/visual presentation.
- Each project cited must have begun and been completed during the 2007-2008 Rotary year.
- No project can be cited in more than one submission.
- Submissions received after the deadline date will NOT BE ACCEPTED.

RI PRESIDENT'S CITATION (Due March 31, 2008):

The purpose of the Presidential Citation is to increase Rotary club involvement in service
activities, while at the same time encouraging Rotarians' personal involvement in serving others and promoting a high standard of Rotary service in the community. The award is revised on an annual basis by the RI president and traditionally highlights the president's theme.
Rotary clubs in good standing are eligible to receive the award. Eligibility requirements are revised on an annual basis, but are generally based on service activities within the Four Avenues of Service.
This RI President's Citation is awarded by Rotary International and is available to each club in the district. RI's President establishes the criteria for this award. DISTRICT GOVERNOR CITATION (Due Mar 31, 2008)
Club Presidents may submit the District Governor Citation Certificate Form, certifying that the club has completed the requirements for the 2007-2008 President citation as detailed in the District Governor Citation Pamphlet.
COMMUNITY SERVICE AWARD (Due April 2, 2008):

Service defines Rotary's character; whether local or international, it promotes Rotary's appeal and visibility in the community, and is the reason Rotary continues to grow.
Community service responds to the needs of a local community. Rotary clubs should determine top priorities for service projects by first learning about a community's needs and assets, and then developing a response that addresses them.
An effective service project:

Responds to a real issue, not an imagined one.

Improves the lives of community members.
-
Incorporates the abilities of those who are served.
-
Recognizes all participants' contributions as important and necessary.
-
Comes from a realistic assessment of resources available.
-
Aims for specific goals and objectives with measurable results.

As business leaders, Rotarians share their skills and expertise through vocational service, one of the Four Avenues of Service. Your vocational service efforts can play a vital role in improving the quality of life for those hardworking members of the community who need direction and expertise. A Vocational project can involve any of a number of vocational service activities:

Mentoring

Career days

Vocational awards, providing business assistance,

Having regular Vocation talks during your Club meeting

Scheduling Vocational Visits

Determining an employer/employee of the year
Your vocational experience can be a valuable resource.

INTERNATIONAL SERVICE AWARD (Due April 2, 2008):

This award involves projects that take place outside of the United States. International projects should identify a specific need that will improve the quality of life and benefit others within that intended area. Some examples are:

Initiate or participate in a matching grant project.

Establish a Sister Club relationship in another country

Sponsor an Ambassadorial Scholars or Group Study Exchange team
-
Host a Rotary Youth Exchange Student
-
Encourage members to personally participate in a project in another country.

Send one or more members to participate in a National Immunization Day. ( NID)

Membership award for retention, development and extension of additional clubs will be based on programs and efforts from July 1, 2007 thru April 30, 2008. A club's membership is a critical component to the success of any club. This involves:
-
Membership growth, development, involvement, training and retention.
-
Social functions.
-
Develop a new member mentoring program and orientation
-
Have regular fireside chats in a member's home.
An award will be given to each of the three club size divisions. The district will be providing "Membership On Target" Certificates to recognize the clubs that are reaching the Membership benchmarks at the end of each quarter.
Membership statistics will be based on figures from July 1, 2007 through April 30, 2008. The award will go to the club in each size division with the greatest percentage net increase in membership (members gained minus members lost, divided by club membership on July 1, 2007).

ATTENDANCE AWARD:
The club in each size category with the highest percentage attendance as reported to the District Secretary for the Rotary year through April 30, 2008 will receive this award.
ROTARY FOUNDATION AWARD (Contributions to TRF thru April 30 2008):
There will be a minimum of two awards within this category.

The first award is for Total Annual Giving and the second award is for Per Capita Giving.
-
Remit all contributions to The Rotary Foundation directly to RI through April 30, 2008.
Total Annual Giving Award: There will be an award for each of the three club size divisions. Total
Annual Giving is the total amount that has been contributed to the Rotary Foundation and doesn't have any bearing on the size of the club.
Per Capita Giving Award: There will be a single award in this category. Total Per Capita Giving is the amount each club has contributed to the Rotary Foundation divided by that club's total membership population. This year, the District 5000 objective is a minimum of $160 per capita. Membership population is defined as the registered number of members that the club began with on July 1, 2007.
ROTARY WEBSITE AWARD (Due April 2, 2008):
This award recognizes those clubs that have worked hard to implement and maintain a quality club website. One of the goals of Rotary is to increase awareness and the Internet helps to facilitate this awareness. Many clubs are using a website for their membership directory, weekly news bulletin, event announcements and club pictures. Websites are able to provide timely information to almost anyplace in the world. Some clubs are using the web instead of mailing newsletters for those members that have missed a meeting. A panel of judges will be put together that will judge the various websites. The judging criteria will be based on:

Timeliness: How timely is the information that is contained on the website and how often is it updated?
-
Content: How is the overall content of the website? Does it provide adequate information to both members and guests that visit the website?
-
Appearance: Is the website attractive and does it invite people to spend more time searching through the site.
-
Navigation: How easy is it to navigate through the website to find the information you might be looking for?
-
Search Criteria: If someone doesn't know the web address, how easy is it to find the website through the various search engines?
Clubs interested in submitting their name for this award must notify their assistant governor by the April 2nd deadline.
DISTRICT 5000 SPIRIT AWARD (Due April 2, 2008):
This award will be given to one new member who has demonstrated exceptional involvement and embodies the true spirit of Rotary. Each club president may select one new member for this award. A new member is one who has been a member for less than one year. Presidents may submit the member's name anytime before the April 2nd deadline to the District Awards Chairperson, Bob Dunning, via email: mauibobd@aol.com or via mail at 760 S. Kihei Rd #502, Kihei, HI 96753. This award will be presented at the District conference on Sunday Evening. It is important that the nominee be present to receive the award.
QUIET ROTARIAN AWARD (Due April 2, 2008):
This award will be given to one member who has demonstrated Rotary involvement in a quiet fashion and normally does not receive recognition for doing good work. Each club president may select one member for this award. Presidents may submit the member's name anytime before April 2nd deadline to the District Awards Chairperson, Bob Dunning, via email: mauibobd@aol.com or via mail at 760 S. Kihei Rd #502, Kihei, HI 96753. This award will be presented at the District conference on Sunday Evening. It is important that the nominee be present to receive the award.
RI SIGNIFICANT ACHIEVEMENT AWARD (Due March 1, 2008):
The purpose of this RI award is to recognize outstanding club service projects. Competition and selection of winning entries takes place at the district level. No club may receive a Significant Achievement Award more than once for the same project
Outstanding club projects that meet the following criteria are eligible to receive the award.
Projects must:
-
Address a significant problem or need in the local community.
-
Involve most or all of the club members in personal rather than merely monetary service.
-
The project also should be commensurate with the club's size and resources.
-
Be capable of emulation by other Rotary clubs.
-
Be currently active or reach a conclusion during the Rotary year for which the award is given.
-
Be carried out by a single Rotary club.

The nomination form must be submitted to the District Awards Chair, Bob Dunning, via email: mauibobd@aol.com or via mail at 760 S. Kihei Rd #502, Kihei, HI 96753. by March 1, 2008 for judging. The District Governor will make the submission to RI. Nomination must be received at RI by March 15, 2008.
RI's FOUR AVENUES OF SERVICE AWARD (Due April 2, 2008):
Note that clubs may submit the nomination at any time during the year until the final RI deadline of June 30, 2008. In order to receive recognition at the District Conference, the Four Avenues of Service Award application must be submitted by April 2, 2008. This award may be presented at the club level at any time during the year.
Recognizing that the strength of the Rotary movement is founded on the service carried out by club members worldwide, this citation program provides a means for Rotary clubs to personally recognize one member of the club for his/her outstanding efforts in the Four Avenues of Service.
Candidates for nomination should be Rotarians in good standing in their clubs that have
consistently demonstrated their support of the Object of Rotary through participation in service activities in each of the avenues of club, vocational, community, and international service. Only one person per club may be nominated per year. Current club presidents and immediate past and current district governors are not eligible.
Clubs nominate one candidate utilizing the RI nomination form. The club president must endorse the nomination form and submit it to RI Headquarters, with a duplicate copy provided to the District Governor.

RI PUBLIC RELATIONS AWARD (Due April 2, 2008):
The purpose of this award is to recognize Rotary clubs that have generated increased awareness and understanding of Rotary through outstanding media coverage or public relations efforts. Competition and selection of winning entries takes place at the district level.
An entry must relate to a single club project, event, or campaign (activities pertaining to just one topic, e.g., drug abuse prevention, literacy promotion) carried out by a single club that demonstrably improves Rotary's visibility and image within the community. Clubs should be able to demonstrate the effectiveness of the public relations efforts by showing one or more of the following results:

Significant media coverage;
-
Increase in community support for Rotary service activities;
-
Public recognition (receiving a community award, for example); and
-
Increased membership.
Public relations efforts must be directed at external audiences (non-Rotarians). Internal education efforts will not be considered. Competition is open to all Rotary clubs. However, district governors may only submit one club's entry per year.

Note that the same submittal provided at the district level, if selected as the overall outstanding district award recipient, will be submitted to RI World Headquarters to meet the May 15, 2008 PR submission deadline. District judging for this category will be performed by the District Awards Committee, chaired by Bob Dunning.
